EMCAS know all too well the difficulties that are faced when trying to provide people with the care they need in order to avoid unnecessary transportation to hospital on the basis of social care requirements.

Emergency adult social care for hospital avoidance is delivered by our well trained and committed team of Community Carers. EMCAS is able to take referrals directly from the ambulance service, A&E, GPs and primary health care providers as well as other services with the aim of providing emergency social care in the community.

Eligibility Criteria for Emergency Care

Any people living in East Lindsey and North East Lincolnshire over the age of 18 that require our care to facilitate a safe and faster hospital discharge, or would prevent  avoidable hospital visits altogether and have been approved our care via the Loacal Authority (LA) This will be accomplished by:

  • Providing 72 hours of initial emergency care to enable a safe and caring return home from hospital or indeed to remain at home with the additional support required.
  • Delivering care and support services to allow for a timely response from other agencies and support services to establish the required longer term needs of the patient post 72 hours.
  • Offering a meet and great service at the home address following a hospital, A&E or intermediate care facility admission.
